• preventative wellness care

All services are provided at the discretion of a licensed medical provider.

Submitting an intake form, scheduling an appointment, or making payment does not guarantee treatment or services.

All services are subject to:

• clinical appropriateness
• provider discretion


No Emergency Services

208 Vitality does not provide emergency medical care.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

Patient Responsibility

You agree to:

• provide accurate and complete medical information
• update your information as needed
• follow medical advice provided

Failure to do so may impact your care.

Payment Terms

All services are cash-pay only unless otherwise specified.

Payment is due at time of services.

Limitation of Liability

To the fullest extent permitted by law, 208 Vitality PLLC is not liable for any indirect, incidental, or consequential damages arising from services provided.

Governing Law

These terms are governed by the laws of the State of Idaho.

Billing

• Memberships are billed monthly
• Payment method must be kept on file

Cancellation

• Membership may be canceled with [30 days] notice
• No partial refunds for unused time

Non-Transferable
